Participation Waiver, Release of Liability & Photo Release

1. Assumption of risk. I understand that participation in The Neighborhood Cup — a recreational, multigenerational sports and civic league — involves inherent risks of physical activity, including but not limited to falls, collisions, equipment contact, weather exposure, and the condition of fields, courts, and facilities, and that these risks can result in injury, illness, property damage, or in rare cases death. I voluntarily accept and assume all such risks, whether or not listed here.

2. Release and waiver. To the fullest extent permitted by Indiana law, I, for myself and my heirs, assigns, and personal representatives, release, waive, and discharge The Neighborhood Cup, Inc., its officers, directors, volunteers, commissioners, referees, member neighborhoods, sponsors, venue owners and hosts (including the City of Carmel and Carmel Clay Parks & Recreation where applicable), and Frissonomics LLC (collectively, the "Released Parties") from any and all claims, demands, or causes of action arising out of or related to my participation in league activities, including claims arising from the ordinary negligence of the Released Parties. This release does not extend to gross negligence or willful misconduct.

3. Fitness to participate. I confirm I am physically able to participate in the activities I register for, have no medical condition that makes participation unsafe, and will withdraw from play if a condition arises. I am responsible for my own medical insurance.

4. Indemnification. I agree to indemnify and hold harmless the Released Parties from claims brought by third parties arising from my own actions during league activities.

5. Photo, video & media release. I grant the league the irrevocable right to use photographs, video, audio, and my name, team, and results captured at league events in league media — website, standings, social channels, recap content, and promotional materials — without compensation. (For minors, this consent is given by the guardian and may exclude the minor's name per Rulebook § 22.3.)

6. Conduct and rules. I have access to the league Rulebook and agree to play under it, including the Code of Conduct (§ 20) and Weather Policy (§ 23).

7. Guardian consent (players under 18). If registering a minor, I certify I am the minor's parent or legal guardian, I make the above acknowledgments and releases on the minor's behalf, and I consent to the minor's participation in youth-eligible sports and roles per Rulebook § 22.

8. Severability. If any provision of this waiver is held unenforceable, the remainder stays in effect. This agreement is governed by Indiana law.
